Why the neck ages the way it does

A heavy, hanging neck is rarely one problem — it's usually a combination of excess fat, loose skin, and weakened muscle banding working together.

Submental Fat

Fat accumulates beneath the chin over time, sometimes independent of overall body weight, creating fullness that no amount of diet or exercise reliably resolves on its own.

Why direct excision, not liposuction alone

Direct submentoplasty involves directly removing the excess hanging skin and fat under the chin and neck, then plicating — surgically tightening and suturing together — the underlying platysma muscle to create a defined neck angle. This procedure focuses exclusively on the neck and does not affect the jawline or lower face laxity the way a full facelift does.

Liposuction alone can reduce submental fat volume, but it treats only one part of the problem. If the platysma bands are loose and the skin has lost meaningful elasticity, removing fat without addressing the muscle and excess skin leaves the underlying cause — the structural laxity — untouched, which is why liposuction-only approaches on a heavy or aged neck often disappoint.

The trade-off is incision location: rather than being hidden around the ear as in a full facelift, the incision sits at the front and center of the neck in a slight "Z" pattern — more effective at correcting heavy laxity, but a different scar consideration than a facelift's ear-hidden incision.

What recovery actually looks like

What about nonsurgical options?

Surgery is the only option to eliminate loose skin, particularly if it hangs or droops. Nonsurgical skin-tightening treatments can offer some improvement, but a direct neck lift more dramatically redefines and smooths the neck's contours. Kybella, a popular injectable, reduces submental fat that causes a double chin — but it doesn't correct excess skin or platysma banding.

Will I have visible scars?

Your neck lift begins with an incision below the chin, positioned so it's difficult for others to see once healed. Dr. Kim works carefully during surgery to minimize visible scarring, and you'll receive scar care instructions to help incisions heal thin and flat.

How is this different from a full facelift?
